Key Responsibilities: Patient Assessment and Support: Conduct initial assessments of patients' mental, emotional, and psychological well-being through interviews, surveys, and discussions. Provide support to patients dealing with chronic illness, mental health issues, or emotional trauma, helping them cope with the physical and emotional aspects of their conditions. Offer guidance and resources to patients and their families regarding treatment options, hospital procedures, and care plans. Counseling and Therapy: Provide one-on-one counseling sessions for patients, addressing their concerns, anxieties, and fears related to their health or treatment. Use therapeutic techniques to help patients develop coping strategies for managing stress, depression, anxiety, grief, or trauma. Develop and implement personalized care plans in collaboration with medical teams to address the patient's emotional and psychological needs. Collaboration with Healthcare Teams: Work closely with doctors, nurses, social workers, and other healthcare providers to ensure comprehensive care for patients. Participate in team meetings to discuss patient progress and collaborate on treatment adjustments or interventions. Ensure that the emotional and mental health needs of patients are integrated into their overall medical treatment plans. Patient Education: Educate patients about the emotional and psychological impact of their medical conditions, helping them understand the importance of mental health in overall wellness. Provide information on available support groups, counseling services, and community resources. Assist patients and their families in understanding treatment plans and how to manage their conditions effectively. Crisis Intervention: Provide immediate emotional support during crisis situations, such as after a diagnosis, surgery, or in case of a medical emergency. Offer counseling for patients experiencing emotional distress, grief, or anxiety related to their health conditions or life changes. Assess the need for referral to other mental health professionals or support systems when necessary. Documentation and Reporting: Maintain confidential records of counseling sessions, progress notes, and treatment plans in compliance with healthcare regulations. Prepare regular reports on patient progress and document any changes in their psychological or emotional status. Ensure that all counseling interventions are properly documented for continuity of care and legal compliance. Advocacy and Patient Rights: Advocate for the emotional well-being and mental health needs of patients within the healthcare setting. Help patients navigate healthcare systems and ensure they have access to the resources they need, including financial support and legal assistance if necessary. Provide emotional support for families and caregivers, helping them understand the impact of the patient’s illness and care needs.
